Paper card sleeve, front with printed text and 'flames' design, with black, orange, red and pink inks. Reverse, printed text in black ink. Handwritten text, blue ink. Disc recording held within transparent plastic internal sleeve. Disc recording made of circular black disc with concentric groove on front and reverse. Circular hole and three circular slits cut in and around centre of disc. Circular paper label around central hole. Label printed with blue and silver inks. White paper sheet held within sleeve, with handwritten, blue ink text and printed ruled lines, blue ink.
