Physical Description
Beige Folder containing two information books (coloured light green and yellow) in one pocket and five reference documents in another pocket. A patient identity card is sellotaped to the front cover (this is mentioned in the booklet). The card has five slots punched into it. The folder itself has text and photographs printed on it.
