The carriage is decorated as a hospital with a banner and people dressed as nurses and injured soldiers. Four men in railway uniform are posing alongside the locomotive and carriage. One man is standing in the coal bunker part of the locomotive. An Australian flag is on top of the carriage.

Year is handwritten at top of cardboard mount. Text on banners hanging from locomotive and carriage, partly obscured by people standing in front of them.
