The Shire of Otway was founded in 1919, a date also commemorated on the medal. The Shire was amalgamated to form the Colac-Otway Shire Council in 1994.

It features a shield surmounted by small shield bearing Southern Cross and supported by emu (not touching shield) and kangaroo, the shield bears a cow, orchard and map depicting rivers, and the sesquicentenary logo.
